Comprehensive Assessment . Session will be established with the representative. Registration is required prior to beginning session. A hold fee of $50 will be applied to your session date. This will apply to your deductible, copay or cost of assessment at the time of session.
If you are unable to attend the session and do not cancel within 3 business days $50 will be non refundable.
